Every idea goes through the same gates. I write the change, run the tests, check that the program's accounts still line up, rehearse it on devnet, publish a verified build, and only then upgrade on mainnet. At most one mainnet upgrade a day. If a gate fails, nothing ships and nothing is written in the journal.
